Processing personal data of patients or clients

Personal customer data are processed by Fysiotherapie de Jong for the following purpose(s):

  • Patient data capture for treatment process
  • Capturing client data for the purpose of physical fitness

Basis for this personal data is:

  • The treatment agreement for physical therapy treatment sessions or physical fitness subscription.

For the above purpose(s), Fysiotherapie de Jong may request the following personal data from you:

  • First name;
  • Insert;
  • Last name;
  • (Business) Phone number;
  • (Business) E-mail address;
  • Gender;
  • Date of birth;
  • Health data;
  • Insurance details;
  • BSN;
  • ID or passport number;

Your personal data will be stored by Fysiotherapie de Jong for the purposes of the aforementioned processing(s) for the period:

  • For the duration of the treatment agreement. After the end of the treatment agreement, we are held to a legal retention period of 20 years in accordance with the Medical Treatment Agreement Act (WGBO).

Processing of personal data from healthcare providers

Personal data of caregivers are processed by Fysiotherapie de Jong for the following purpose(s):

  • Exchange of patient data for the purpose of treatment.

Basis for this personal data is:

  • Referral from general practitioner or medical specialist;
  • Informing a.k.a. direct access physiotherapy
  • Intercollegiate consultation

For the above purpose(s), Fysiotherapie de Jong may request the following personal data from you:

  • First name;
  • Insert;
  • Last name;
  • E-mail address;
  • Care email address
  • Phone number;
  • AGB code;

Your personal data will be stored by Fysiotherapie de Jong for the purposes of the aforementioned processing(s) for the period:

  • For the duration of the patient's treatment agreement. After the end of the treatment agreement, we are held to a legal retention period of 20 years in accordance with the Medical Treatment Agreement Act (WGBO).

Provision to third parties

We may provide the information you provide to us to third parties as necessary to carry out the purposes described above.

For example, we use a third party for:

  • Taking care of digital record keeping (EPD);
  • Taking care of the (financial) administration;
  • Providing patient satisfaction survey retrieval;
  • Providing anonymized data to the National Database of Physical Therapy;
  • Processing claims or bank transactions;

We never pass on personal data to other parties with whom we have not concluded a processing agreement. We will of course make the necessary agreements with these parties (processors) to ensure the security of your personal data. Furthermore, we will not provide the data you have provided to other parties, unless this is legally required and permitted. An example of this is when the police request (personal) data from us as part of an investigation. In such a case we are obliged to cooperate and hand over this data. We may also share personal data with third parties if you give us written permission to do so.

Within the EU

We do not provide personal data to parties located outside the EU.

Minors

We only process personal data of minors (persons younger than 16 years of age) if written consent is given by the parent, guardian or legal representative.

Retention period

Fysiotherapie de Jong retains personal data no longer than necessary for the purpose for which it was provided or required by law.

Security

We have taken appropriate technical and organizational measures to protect personal data about you from unlawful processing, for example, we have taken the following measures;
