    Physiotherapy payment terms

    1. These payment terms are part of the treatment agreements between you as a patient and the physical therapist. These payment terms can be found on our website and are available at our practice and can be taken away by the patient/client.
    2. Appointments that cannot be kept should be cancelled no later than 24 hours before treatment. For Mondays, this applies before 9:00 a.m. In case of no cancellation or cancellation within 24 hours before the appointment, the physical therapist reserves the right to charge the patient 75% of the rate of the intended treatment.
    3. If you have to pay for the treatment yourself, you should pay by pin immediately after the treatment in our practice.
    4. The physical therapist's bills for fees, expenses, reimbursements and supplies, must be paid within 14 days after receipt of the invoice. If payment is not made within the said period, the patient shall be in default as of the ninth day after receipt of the invoice.
    5. As soon as the patient is in default, the physical therapist is entitled to take collection measures and charge collection costs. These costs are due on the basis of the legal provision regarding collection costs (Article 6:96 of the Civil Code). In addition, Coverage contractual and/or statutory (commercial) interest is then claimed from the date you are in default.

     

    Payment Terms Fitness

    Membership and Payment

    1. Membership at Fysiotherapie De Jong is valid for a period of 3/12/18 months, depending on the type of subscription purchased. After the initial period expires, membership is automatically renewed monthly unless cancelled in writing with one month's notice.
    2. The monthly membership fee is as per signed subscription form and depends on the type of subscription purchased. The rate is indexed annually and is listed on our website.
    3. Payment is made monthly by direct debit. Late payment may result in denial of access.

    Cancellation and Break

    1. Termination of membership may be submitted in writing with one month's notice.
    2. Membership may be temporarily paused with prior written notice. A €15 administration fee will be charged for restart after a break (rate 2025).

    Access and Opening Hours

    1. Fitness opening hours can be found on our website and in the Technogym app. Different opening hours will be announced.

     

    General conditions

    Rules of conduct

    1. Respect staff and fellow athletes. Discrimination, harassment and aggressive behavior will not be tolerated.
    2. Keep the practice clean and tidy. Clean up used materials. Dispose of trash in the trash cans provided.
    3. Athletes and visitors must follow the instructions of the staff at all times.
    4. Upon entry, the athlete must sign in at the kiosk, next to the PC in the exercise room, using the wristband. If the wristband is lost, a new one must be requested at Coverage .
    5. Inappropriate behavior should be reported immediately to a staff member.
    6. Wearing jewelry is not recommended for safety reasons.
    7. Only clean shoes should be used in practice.
    8. During peak times, no training equipment should be used for more than 15 minutes at a time.
    9. No visual material, in any form, is allowed within the practice.
    10. The practice must be vacated no later than the applicable closing times, see our access and opening hours on our website.
    11. Patients who train under the supervision of a physical therapist have priority over fitness members when using the machines.

    Hygiene

    1. Always use a towel on fitness equipment.
    2. Clean the equipment after use.
    3. Leave the toilet tidy after use.
    4. If you have comments about hygiene or cleaning, report it to a staff member.

    Safety

    1. Follow instructions for using equipment and ask for help if needed.
    2. Report broken or defective equipment to a staff member immediately.
    3. Part of the intake is a health questionnaire. If you have any doubts about medical issues, consult with your doctor if necessary.
    4. Report medical problems to a staff member immediately.
    5. First aid facilities are available. Report injuries/injuries to a staff member immediately.

    Liability

    Fysiotherapie de Jong is not responsible for loss or theft of personal property.

    Failure to comply with these house rules may result in a warning or that management will deny you access to the practice.
